If your appointment with any of our practitioners is to be paid by your insurance company, we MUST receive the approval in writing from your insurance provider for that service prior to your appointment. We are unable to proceed with your appointment if we have not received the appropriate approval first.
When you make the appointment, you need to advise that this is an insurance claim appointment and your must organise for the insurance company to send through the approval. Please check with us that we have the approval in place before you come. It is not up to us to advise you that this has come through.
Initial appointments for Insurance Claim cases are for one hour and the consult fee, which includes the report for the insurer, is $360. The written report is a detailed assessment and treatment plan.
The details we require from you (apart from the approval) are the name of your case manager, their phone number and the email address where we send the invoices.
If you have any paperwork regarding your claim, please bring this in with you to your appointment.
MY AGED CARE
We require the details of the name of your service provider, the name of your case manager, their phone number and the email address where we send the invoices.
If you have any other paperwork regarding your plan with My Aged Care, please bring this in with you to your appointment.
